Employee Pre-Boarding Information

Welcome to Broward Health and Congratulations on Your Offer for Employment

This page is designed to give prospective employees an idea of what to expect as you begin your journey with Broward Health. Please ensure you have all the necessary documentation when attending your pre-employment processing meeting. Per Broward Health policy, all prospective employees must complete the pre-employment process before they can begin working. Any prospective employee file that is not fully completed by close of business the Thursday prior to their orientation date cannot start on the following Monday morning.

The onboarding process takes about 2-3 hours to complete depending on your specific circumstances, therefore to help reduce the time we have attached our forms below. Please print and complete the forms below and bring them with you to your onboarding appointment.

Step 1

Offer and Scheduling Pre-Employment Processing Appointment

Once an offer has been made and accepted, a Human Resources Representative will set a pre-employment and employee health appointment. Each applicant is extended a conditional offer of employment and must pass all aspects of the pre-employment process.

Step 2

Background Check Authorizations

A pre-employment background investigation will be conducted on candidates for whom an offer of employment is being considered. You will receive the electronic authorization via email for the background screening. Please complete the form in order to start the background check process. In addition, Human Resources will also conduct two employment reference checks as a part of the pre-employment process.
Employment Eligibility Verification

Federal law requires that Broward Health document and verify the identity and employment authorization of each new employee (regardless of immigration status) hired after November 6, 1986 to ensure authorization to work in the United States. Documentation of eligibility to work in the United States will be required as a condition of employment. Candidates must present documentation proving eligibility for employment in the United States. Please see the Form I-9, instructions, and acceptable document list below. Broward Health participates in E-verify.

Complete all items in the left column to bring with you to your HR representative. Review and be familiar with items in the right column.

Step 4

Pre-Employment Processing Appointment and Documentation Review

During the pre-employment processing meeting, the applicant will review his/her new hire paperwork with a Human Resources Representative. The new hire will also receive information on General Orientation.

Documents to bring with you to your pre-employment meeting:

  • I-9 documentation (see list of acceptable documentation)
  • Original Social Security Card (For Payroll Purposes)
  • Proof of highest level of education completed (Original Documents or Official Transcripts)
  • Any licensures/certifications required for the position

Step 5

Pre-Employment Medical Examination

Broward Health Pre-employment medical evaluation which includes a medical evaluation and drug alcohol screening test by our Employee Health staff (at no charge to the applicant) prior to your start date.

  • Notice of Privacy Practices for Protected Health Information
  • Employee Health Brochure

At the Employee Health Office, the applicant will:

  • Have a TB test administered 
  • A chest x-ray will be completed if a previous TB test was positive.
  • A two-step TB test is required if the applicant does not have documentation of a negative test within the past year or if new to the health care industry.
  • If you have had a positive TB skin test in the past or been treated for TB, documentation of this history from a provider is required. If you do not have this documentation, a skin test will be placed on the day of your physical.
  • A urine sample will be collected during the physical for a drug test.
  • Bring your glasses or wear your contact lenses on the day to your physical to facilitate your vision screen.
  • Complete the medical health history questionnaire.
  • Review influenza vaccine history.
  • Receive a TB mask fit testing questionnaire (dependent on position at the facility).
  • Receive a request for documentation of titers to include varicella, MMR, and Hepatitis B or possibly have laboratory testing completed.
  • If you have a recent surgical procedure, you may be required to provide a note from your provider releasing you to resume work duties.

The applicant returns to Employee Health normally two to three days later to:

  • Have the TB test checked
  • Review need for two step TB testing
  • Complete a TB mask fit testing if needed
  • New employees will be provided with a boarding pass. This boarding pass is used for admission into orientation.

Step 6

Employee Benefits

You will receive an enrollment guide, instructions, log-in, and password to enroll in Employee Benefits from HR when you pick up your ID badge. For information about the available benefits, including summaries and plan documents, please visit the Employee Benefits page.
